Abstract:	This paper was first published online on 24 March 2013 (DOI: 10.1002/navi.28) and appears in the Spring 2013 issue of the journal (Volume 60, Issue 1, pp. 71-83). Subsequent to its publication online and in print,we discovered an error in equation numbering.
