Team Application Forms
A team application form is required for entry into the competition. A Release and Consent Form must be completed and signed for each student or adult that will participate in the competition. Each team is required to complete one Indemnification Agreement form.
Please send your completed application and all signed waivers to Megan Andrews at mandrews@ion.org or fax them to 703-366-2724.
